Upload instructions:
To upload forms go to your account page
->Under the player name click on the left hand button (looks like an address card)
->Scroll down to registration history & click view details
->Scroll down to documents & click on required at roster on each relevant document
There may be a shortcut which I will keep looking for but this is how I currently know how to do it
